Lake Township's policy allows a request without a written form, name, or stated reason. The office may ask for optional detail to locate the record and may provide existing paper, existing medium, or another reasonably duplicable format. Its first five paper pages are free, with later pages at five cents, subject to the policy's mailing and supply terms.
